Meet Janelle Smith, DPT, OCS
Janelle earned her undergraduate degree in Kinesiology from the the University of Illinois and her Doctorate of Physical Therapy from St. Ambrose University. She has over a decade of experience as an Orthopedic Certified Specialist and has provided years of mentorship to new therapists through an orthopedic residency program. She is a dedicated physical therapist offering a personalized and seamless experience that goes above and beyond traditional therapy. Beyond teaching and training other therapists she has gained training and provides dry needling, cupping, blood flow restriction, and instrument aided tissue mobilization.
Meet Kate Miller, DPT, OCS
Kate joined the Evolve Physical Therapy and Sports Medicine in 2025. Kate graduated from University of Alabama with her Bachelor's Degree and her Doctorate of Physical Therapy degree from Emory University. Kate completed a post doctorate orthopedic residency program prior to gaining her Board Certificaiton in Orthopedics. Kate offers Dry Needling and is certified to work with pregnancy and post partum patients. Kate brings a passion for empowering patients through movement, strength, and education.
Meet Dannielle Jenkins
Dannielle joined the Evolve Physical Therapy and Sports Medicine in August 2026. Dannielle graduated from Northern Illinois University with her Bachelors in Science in 2004 and went on to earn her Masters in Physical Therapy in 2007. Dannielle was a division I gymnast for NIU, competing four years as an all around gymnast. Dannielle brings 19 years of orthopedic and sports medicine experience to Evolve, with a special interest of post operative care for the shoulder and knee diagnoses. Outside of work Dannielle enjoys spending time with her husband and three children, who are involved in basketball, volleyball, and gymnastics. In her free time, Dannielle enjoys biking, running, and strength training.

Meet Mark Brauweiler, DPT, OCS, CMT, CSCS
Mark joined the Evolve Physical Therapy and Sports Medicine in March 2023. He received his Doctor of Physical Therapy (DPT) degree from St. Ambrose University in 2015. He has been board-certified in orthopaedics (OCS) through the ABPTS since 2019; completed a year-long post-graduate Certified Manual Therapist (CMT) program in 2018; and has been a Certified Strength & Conditioning Specialist (CSCS) through the NSCA since 2015.
Outside of work, Mark loves spending time with his wife and kids. He is also a fan of Notre Dame, as well as the Chicago Bears, Cubs, Bulls, and Blackhawks. He likes to stay active through running, working out, playing volleyball, and golfing.
Meet Oliver Dela Rosa, OTR/L
Oliver joined the Evolve Physical Therapy and Sports Medicine in March 2026. He received his Occupational Therapy degree from Mount Mary University in 2018. Oliver has nearly a decade of experience, specifically focusing on hand and upper extremity conditions including post operative shoulder care. Oliver's vast knowledge and expertise will help you maximize your potential.
Meet Austin Miller, DPT
Austin earned his Doctorate of Physical Therapy from Emory University and is certified in Selective Functional Movement Assessment. He has a background in exercise science and kinesiology with a Bachelor’s degree in Exercise Physiology from the University of Miami. With a personal experience of recovering from a low back injury to becoming a nationally ranked powerlifter, he is passionate about helping people get back to the sports and activities they love. In addition, Austin has an extensive professional history of working closely with doctors to help post-op patients recovering from their surgeries. In his free time you can find Austin searching for great restaurants and hanging with his two cats Fox and Spirit.
